Student-Athlete Registration is available on FinalForms for all students in grades 7-12. Below are steps for parent and student registration. The only form the Athletic Department requires is the OHSAA Pre-Participation Physical Form, located here. The physical form must be completed by the healthcare provider and, by law, we are required to have a signed copy on file. Once a physical has been received, we will input the expiration date into FinalForms and you will receive automatic notifications when forms need to be updated/completed. 

STEPS

1. Read Parent Registration Step-by-Step Guide.

2. Log onto FinalForms and complete paperwork with digital signatures. **NOTE: Students must sign forms as well.

3. Schedule a physical.

4. Return signed OHSAA Pre-Participation Physical form to the Athletic Department.
